From the moment you reach out, our team is focused on one thing: making sure your loved one lands in the right environment. Here's what the EverRise experience looks like.

A warm, structured home environment

Our homes are real homes — not facilities. Residents have private or semi-private rooms, shared common spaces, home-cooked meals, and a consistent daily routine that supports stability and wellbeing.

Person-centered care plans

Every resident receives an individualized care plan developed with input from the resident, their family, and any existing support providers. Plans are reviewed regularly and updated as needs change.

24/7 trained staff support

Our staff are on-site around the clock. Whether it's medication management, personal care, or simply being present, someone is always there.

Community and connection

We believe belonging matters. Residents participate in community outings, in-home activities, and social events designed to foster friendship, purpose, and joy.

Family communication

We keep families informed and involved. You'll have a direct line to our care team and are always welcome to visit, call, and participate in care planning.

Placement process

How placement works

Submit an application

Start by completing our online application or calling our intake team. We'll gather basic information about the individual's needs, current situation, and program interest.

Intake assessment

A member of our team will conduct a brief assessment — by phone or in person — to understand the individual's support needs, preferences, and goals. This helps us determine the best fit.

Home tour

We invite prospective residents and their families to tour the home, meet current residents and staff, and get a feel for the environment before making any decisions.

Placement decision

Once a match is confirmed, we work with the family, case manager, and any funding sources to finalize paperwork and set a move-in date. We move at your pace.

Move-in & transition support

Our team provides hands-on support during the transition period to help new residents settle in comfortably. We stay in close contact with families during the first 30 days.
